To apply for Conditional Membership, please fill out and submit this form any time in April. We won't be making any decisions about applications until after April 30; we will let you know as soon as possible after the closing date.
If your application is accepted, we'll send you an invoice for the $60 Conditional Membership fee.
Submission of your application means that you agree to the following statements:
1. Random Draw.
"I understand that there could be a random draw of applications and that submission of this application does not guarantee me an opportunity to become a Conditional Member."
2. Conditional Membership Requirements.
"I understand the requirements of Conditional Membership."
3. Full Membership Eligibility
"If my application is selected and approved, I understand that I must complete all the requirements of Conditional Membership before being eligible to apply for full Membership. Once eligible, I understand that I must submit an application for full Membership, which must then be approved by the AOC Board of Directors before I am presented to the Membership for acceptance."
4. Albany Obedience Club, Inc. Waiver, Assumption of Risk and Agreement to Hold Harmless.
"I understand that attendance at an Albany Obedience Club Inc. event is not without risk. In consideration of this, I hereby agree to indemnify and hold harmless the Albany Obedience Club Inc., its officers, members, and agents from any and all claims while on the grounds or the surrounding area thereto. I agree to abide by the policies and constitution of the Albany Obedience Club Inc., copies of which are also available on the club website."
